JONESBORO — It’s that time of year for parents of soon-to-be kindergarteners.
Clayton County Public Schools has set aside a week in April for parents to register their children at their zone elementary school for the 2019-2020 school year. Students must be 5 years old on or before Sept. 1 to enter kindergarten.
Parents can register April 22 through 26 from 8:30-11 a.m. and noon to 2:30 p.m. at each elementary school.
To register, parents will need several documents. The following is a list of what is needed:
• Proof of Age. A certified copy of a birth certificate or a federal, state, county or school document with a date of birth. Examples include passport, adoption record or religious record signed by an authorized religious official or immigration documents. Parents can obtain a birth certification for their Georgia-born child by visiting http://gta.georgia.gov/rover.
• Proof of authorized person to enroll. The following are authorized to enroll students: parent (natural or adoptive), legal guardian, foster parented appointed by a state agency or sponsor for approved International Exchange Program. Those enrolling must present of of the following forms of identification: driver’s license, state identification card, passport or other official photo identification.
• Proof of residence. One of the following are accepted and must include an address: non-contingent sales contract, current lease or rental agreement, current residential property tax statement or bill, current warranty or quit claim deed, current home purchase agreement, current homeowner’s insurance policy. Additionally, a current gas bill, water bill or electric bill must be presented.
If a student’s family is living in the home or apartment of another person, the following information is required for enrollment: notarized third-person affidavit of residency (available to school) and a signature of the person whom the family is living, signature of parent and apartment manager’s signature if applicable. Two forms of proof of residency will also be required.
• Immunization/Health Certificates. A valid certificate of immunization (Ga. Department of Health form No. 3231) or a notarized affidavit of religious exemption ( Ga. Dept. of Health form No. 2208). A vision, hearing, dental and nutrition screening (Ga. Dept. of Health form No. 3300, rev. 2013) is also required.
