Observed employee rinse hands inside produce sink without soap and no paper towel. CA: Employees must wash hands in handwashing sink that is properly equipped. COS: Infomed PIC. Employee washed hands properly in designated handsink with soap and dried with paper towels.  Corrected On-Site.  New Violation.

Observed several containers of raw and ready to eat foods stored inside reach in coolers, freezers and walk in coolers uncovered. CA: Cover food to prevent contamination. COS: Informed PIC to cover food.  Corrected On-Site.  Repeat Violation.

Observed employee wash dishes in 3 compartment sink and place on shelf for air drying without sanitizing first. CA: Food contact equipment must be properly washed, rinsed, and sanitized using chlorine solution of 50-100ppm (facility uses chlorine bleach). COS: Informed PIC of proper way to clean food contact equipment.  Corrected On-Site.  New Violation.

Observed ready to eat food inside broken plastic container with large hole. PIC stated they intentionally break holes in plastic containers for convenience. CA: equipment shall be finished to have a smooth, easily cleanable surface. Materials shall be resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition. COS: informed PIC  Corrected On-Site.  New Violation.
