Observations and Corrective Actions

PIC could not provide documentation or otherwise satisfactorily demonstrate during inspection that food employees and conditional employees are informed of their responsibilities to report to management information about their health an activities as it relates to diseases that are transmissible through food. PIC did not have information on file He had to go to his apartment to retrieve the information. COSPIC educate staff on fod relate illnesses. COS: Red book given to PIC.  Corrected On-Site.  Repeat Violation.

Observed cooked chicken improperly cooling and pass the 4 hour cooling period. COS: PIC discarded food items.  Corrected On-Site.  Repeat Violation.

Observed several food items being stored and used passed the date marked date, not being discarded at correct date. (PIC advised that they forgot to change date. Did not discard food items.  New Violation.

Observed uncovered food being stored under leaking pip inside of walk in freezer.  Repeat Violation.

Observed employee preparing plates without hair restraint at time of inspection. COS: employee put on a hair restraint during inspection.  New Violation.

Observed no current inspoection report posted at time of inspection. PIC advised that they dont post in the store due to mgt. CA: The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. Observed the establishment did not have previous score posted.  Repeat Violation.

Observed a non approve stove being used in establishment at time of inspection.  New Violation.

Observed several food and non food contact surfaces of cooking equipment and non food contact surface covered in accumulation.  Repeat Violation.
